For referrers
In order to provide the best service and clinician-client match to your patients, I am happy to discuss possible referrals via phone on 0499 583 357 if that is your preference. If I am with a client and not able to answer, please leave a message and I will return your call as soon as possible.
Alternatively you can send a fax with your letter of referral and MHCP to the following number: 02 9182 5943.
As per Medicare requirements, a valid referral from a GP requires the following information to be included in a letter:
* Patient’s name, date of birth, address
* Referral date
* The number of sessions that a patient is being referred for: Initial Better Access items: 6 sessions or less if desired. A patient can claim a maximum of 10 sessions per calendar year under this program with a maximum of 6 sessions per referral.
* Reason for the referral is stated (the presenting problem)
* State that a Mental Health Care Plan is in place
* Signature and provider number of referring GP
At present the usual referral pattern is for 6 sessions, followed by an additional 4 under a new letter
of referral.
Current advice from Medicare states that the Mental Health Care Plan is a treatment plan and is not a referral. As such, a psychologist cannot provide services upon receiving only the Mental Health Care Plan and requires a valid letter of referral.
Please note I do not provide services under Workcover.
